Please read and sign this General Release Statement

I, the below-identified person, hereby make application for membership and training in those programs sponsored by Jacarrie Kicks For Kids, St. Marcus Lutheran School, all participating organizations, and its affiliates conducted at 2215 N Palmer St. Milwaukee, WI 53212.  All statements cited below are true and correct to the best of my knowledge.  Upon acceptance, I pledge to obey all rules and regulations set forth for my protection and the protection of others.

I understand that the study and participation in basketball, baseball, soccer, football, art exploration, urban gardening, martial arts, drumming and/or dance involves vigorous physical activity, falls, throws, punches, kicks, jumps, full body movement and other activities which contain danger and risk of injury and that there is inherent danger in the martial arts, the above aforementioned sports,  and dance.  Nevertheless, I voluntarily elect to accept all risks connected with my child’s participation in all programs provided, taught, conducted, or associated with Jacarrie Kicks For Kids, St. Marcus Lutheran School, and all participating organizations, and its affiliates. 

I HEREBY RELEASE, WAIVE, DISCHARGE, AND COVENANT not to sue Jacarrie Kicks For Kids, St. Marcus Lutheran School, all participating organizations, and its affiliates and its owners, officers, agents, employees, instructors, students, participants, martial arts association, sanctioning organization or any subdivision thereof, premises owners, promoters, sponsors, lessees of premises used to conduct events and each of them, their officers, employees, instructors, students, all the purposes herein referred to as “Releasees” from all liability to the undersigned, his personal representatives, assigns, heirs, and next of kin for any and all loss or damages, and any claim or damages or demands therefore on account of injury to the person or property of, or resulting in the death of the undersigned, whether caused by the negligence of the releasees or otherwise while the undersigned is in or upon the premises of Jacarrie Kicks For Kids, St. Marcus Lutheran School, participating organizations, and its affiliates and/or studying, participating, competing, officiating, instructing, observing, working for, or for any purpose participating in the activities conducted at Jacarrie Kicks For Kids, St. Marcus Lutheran School, participating organizations, and its affiliates.

The undersigned has read and voluntarily signed the release and waiver of liability and indemnity agreement and further agrees that no oral representations, statements, or inducements apart from the foregoing written agreement have been made.
